Requesting time off from work for a personal matter. In your Email, Explain:
- Your name and position.
- The date(s) and duration of the requested time off.

Dear Mr John,
I am writing to request some time off from work for a personal matter. My name is ABC and I am a Team Leader in the Sales department at Departmental store.
I would like to request time off from work starting on 10-05-2023 and ending on 15-05-2023. This would be a total of 5 days. During this time, I will need to take care of a family matter.
I understand that taking time off can cause some inconvenience for the team, and I apologize for any disruption this may cause. I will do my best to ensure that my work is up to date and that I hand over any outstanding tasks to a colleague before I leave.
If there is anything I can do to make the transition easier, please do let me know. I would be happy to provide additional training or assistance to my colleagues before I leave to ensure that my absence is as smooth as possible.
