Dear Mr Max,
I’m writing because my latest health insurance claim was turned down. I put in a claim for an acute appendix surgery I had, but it was turned down.
After reading the rejection letter, I know that reason for rejection, is the claim wasn’t covered by my insurance plan. But I would like to politely ask that this choice be looked at again.
I’ve looked over my policy carefully and think that the services I got are covered by it.
I understand that there may have been some confusion or misunderstanding about my claim, and I’d like your help to clear this up. I’d like to ask you to look at my claim again and decide whether or not to approve it.
